an electronic health record-based practice and provide outstanding
customer service and patient care skills. We are seeking a qualified
NP candidate to join our team who will work in our Lonetree outpatient
clinics and surrounding areas and will care for Colorado Kidney care
office patients as well outpatient dialysis patients. All candidates
must be able and willing to travel to different work locations
regularly; reliable transportation is required.
We offer competitive pay range; annual salary offered at
$103,000 to $106,000 for full time exempt position, commensurate
with qualifications and experience. Excellent benefits package
offered.
Employment at Colorado Kidney Care is contingent on successful
completion of pre-employment screening and background check. This
position will remain open until filled.
Responsibilities and Duties
:
- To round, assess and evaluate as directed on each ESRD patient in
the dialysis units to which you have been assigned. ESRD patient
duties include: chair-side visits and charting of each visit,
filling of prescriptions both chair-side and by phone, contacting
referring physicians regarding clinical issues, assisting with
arrangement of tests when assistance is needed from the dialysis
staff to clarify medical issues, and completing annual History and
Physicals on ESRD patients as needed.
- Evaluate plan of care, adjust as indicated.
- Perform in-person office visits and tele-health visits on
follow-up patients with CKD.
- Perform office consultations on specified hospital follow-ups who
need prompt office follow-up.
- Patient education both in the office and via phone calls to
communicate physician instructions regarding; medication changes,
requested diagnostic follow-up/labs, diet changes, next visit, etc.
- Diabetes education including diet, exercise, monitoring,
medication management, problem solving, coping strategies, and
co-morbid risk reduction strategies.
- Entry, Review, Refill and Maintenance of medications in EHR
- EHR software patient care data entry must be accurate and timely;
you are expected to document all patient care interactions and
pertinent medical record information in the EHR charts. This
includes both in person office visits as well as phone calls with
the patient.
- Answering and triage of phone calls from patients seeking
advice/clarification that the office staff cannot answer.
- Maintenance and upkeep of incoming messages via EHR and email;
this includes timely responding and follow up
- Assist with patient referrals and coordination of care regarding
dialysis, transplant, access, and dietary services.
- Maintain contact with patients to ensure anticipated therapeutic
objectives are being met (i.e. weight, BP, diet, lab work) and
communicate with treating nephrologist regarding objectives.
- Assist physicians with EHR documentation by annotating physicians
completely on all interactions and patient care issues regarding
their patients.
- Cross coverage of other duties as needed, and other duties as assigned
